How we handle information collected through this website.
Effective date: October 2026
Chronos Clinic ("Chronos," "we," "us") is an orthopedic longevity practice in West Los Angeles, California, operating under the medical direction of Omar Rahman, MD, MBA. This policy explains what information we collect through chronosclinic.health (the "Site"), how we use it, and who helps us operate the Site.
We keep two kinds of information apart:
Please do not submit detailed medical information through the Site's contact or consultation request form. That includes diagnoses, medications, medical history, imaging results, or other sensitive health details. We will ask for clinical information through the secure intake process when it is needed. Do not use the Site for emergencies; if you are experiencing a medical emergency, call 911.
Contact information you submit. Your name, email address, and phone number when you request a consultation or contact us.
Consultation and appointment inquiry details. A general reason for contacting us, an optional brief note, your preferred visit format, and the days and times you prefer to be contacted.
Basic website and device information. Like most websites, our hosting provider automatically records technical details when pages are requested, such as IP address, browser and device type, the pages requested, referring page, and date and time. We use this to deliver, secure, and troubleshoot the Site.
Cookies and similar technologies. See "Cookies and Your Choices" below.
Payment information. Consultation and membership payments are handled by our third-party payment processor, Square, on Square's own checkout pages. Square collects your card details directly. We do not receive or store your full card number; we receive confirmation of the payment and related transaction details, such as your name, email, amount, and date.
Clinical information through secure intake. After scheduling, patients may submit clinical information through our patient intake form hosted by Jotform. That information is collected outside this Site's public forms and becomes part of the clinical record.
We use website information to respond to your inquiry, schedule and confirm consultations, process payments, communicate with you about your request and our services, operate and secure the Site, and meet legal and record-keeping obligations. We do not sell your personal information, and we do not share it with third parties for their own marketing.
The Site currently uses only technologies needed to deliver the Site and its forms. We do not use advertising pixels, retargeting, session recording, or heatmap tools. We are adding a consent-management tool, Cookiebot, so that any optional cookies, such as website analytics, load only after you agree to them where consent is required. Once it is active, you can review or change your choices at any time with the "Cookie Settings" link in the footer of every page.
If your browser sends a Global Privacy Control (GPC) signal, we treat it as a request to opt out of the sale or sharing of your personal information and of any marketing technologies. Our pages for consultation requests, payments, and patient consent are set up so that marketing and session-recording tools do not load on them.
We use service providers to run the Site and the practice. They may process information on our behalf only to provide their services to us. Depending on your interaction with us, these may include:
When you leave the Site to use Square's checkout or Jotform's intake form, those pages are operated by those companies and are also subject to their own privacy policies.
Submitting a form on this Site does not create a physician–patient relationship. Once you become a patient, your health information is maintained in our clinical records and protected in accordance with applicable law, including the California Confidentiality of Medical Information Act and, where applicable, HIPAA. Patients receive a separate Notice of Privacy Practices describing how clinical information is used and disclosed.
We use reasonable administrative, technical, and physical safeguards for the information we hold. No website, email, or internet transmission is completely secure. Please keep sensitive health information out of ordinary email and website forms.
We keep website inquiry information for as long as needed to respond to you and maintain business records, then delete it or keep it only as required by law. Clinical records are retained for the periods required for medical records.
Depending on where you live and the laws that apply, including California law, you may have the right to request access to, correction of, or deletion of personal information we hold about you, and to opt out of the sale or sharing of personal information. We do not sell or share personal information as those terms are defined under California law. We will not discriminate against you for making a request. To make a request, email us at the address below. We may need to verify your identity before responding.
The Site is not directed to children under 18, and we do not knowingly collect information from them online. Care for minors, where appropriate, is arranged directly with a parent or guardian.
We may update this policy from time to time; the effective date above reflects the latest revision. Questions or privacy requests: care@chronosclinic.health, or Chronos Clinic, 11710 Wilshire Blvd, Los Angeles, CA 90025.
