Rotator cuff tendinopathy — irritation and degeneration of the cuff tendons without a full tear — is one of the most common causes of nagging shoulder pain in active adults. It often gets stuck: rest helps briefly, generic physical therapy plateaus, and the shoulder never fully comes back. Chronos treats it as a tendon problem that needs the right inputs, not just time off.

Is this you?

How we approach it

Care runs through the Tendon Optimization Program. Depending on your evaluation, a plan may include progressive, tendon-specific loading; treatment modalities such as shockwave and laser/photobiomodulation sequenced to the phase of healing; and, where appropriate, orthobiologic and regenerative options — including peptide and emerging cellular (stem cell–derived) approaches, and high-dose PRP where appropriate — discussed individually and under informed consent. Many are investigational and not FDA-approved; whether any is reasonable for you is a clinical decision, not a guaranteed outcome. If imaging shows a tear that genuinely needs surgical repair, we will tell you.

Common questions

Can rotator cuff tendinopathy heal without surgery?

Frequently, yes. Most tendinopathy and many partial-thickness problems are managed without surgery when the loading and recovery inputs are right. Full, retracted tears are a different situation.

Does shockwave therapy help rotator cuff pain?

Extracorporeal shockwave therapy is an evidence-supported option for certain chronic tendinopathies and may be part of your plan when appropriate.
