Chronos Clinic offers telehealth visits for selected services, including imaging review, second opinions, initial triage, and certain longevity consultations. This notice explains what telehealth involves so that you can give informed consent before a telehealth visit, consistent with California law.

What Telehealth Is

Telehealth means delivering health care services using interactive audio and video technology when you and your physician are not in the same location. Your telehealth visit with Dr. Rahman is a real clinical encounter, documented in your medical record like any office visit.

Benefits and Limitations

Telehealth improves access and convenience, and is well suited to reviewing imaging, discussing results, and planning care. It has limits: a physical, hands-on examination is not possible, and some conditions cannot be fully evaluated remotely. If Dr. Rahman determines that your situation requires an in-person examination, imaging, or urgent care, you will be directed accordingly. You may be asked to complete an in-person visit before certain treatments are initiated.

Your Rights

Your participation in telehealth is voluntary. You may withhold or withdraw consent to telehealth at any time without affecting your right to future care. The laws that protect the confidentiality of your medical information apply to telehealth visits. You have the right to know the cost of a telehealth visit before it occurs; Chronos is a cash-pay practice and fees are disclosed in advance.

Technology and Privacy

Telehealth visits are conducted over secure, encrypted video platforms. No visit is recorded without your explicit consent. As with any internet-based technology, there is a small risk of technical failure or unauthorized interception despite reasonable safeguards; in the event of a technical failure, the visit will be completed by phone or rescheduled.

Emergencies

Telehealth is not appropriate for emergencies. If you experience a medical emergency during or after a telehealth visit, call 911.

Consent

By scheduling and participating in a telehealth visit with Chronos Clinic, you acknowledge that you have read and understood this notice and consent to receiving care via telehealth.
