A structured program for chronic tendon pain that has not resolved — built for the tendon that is stuck.
Chronic tendinopathy is a frustrating diagnosis. The injury is not acute. There is no torn structure on imaging that a surgeon would repair. Physical therapy has been tried — sometimes once, sometimes three or four times — and the result is a tendon that is better than it was at the worst, but not better than it was before the injury.
Eight to twelve weeks organized around three things: modality-driven reactivation of the healing response, targeted mechanical loading at the right dose and phase, and systemic support for the biological environment.
Extracorporeal shockwave therapy, photobiomodulation, Class IV laser, and TECAR are each selected and sequenced based on the phase of recovery and the specific tendon. Delivered through our partnership with Dr. Michael Sheps in Brentwood.
Tendons heal under load, not under rest. Isometric, heavy-slow resistance, or eccentric loading depending on the tendon and phase. Coordinated with a physical therapist when appropriate — a deliberate departure from the passive-care-only model that causes chronic tendinopathies to stagnate.
Metabolic optimization, systemic inflammation modulation, and peptide guidance discussed in the context of the longevity consultation when appropriate.
Return-to-play is not when the tendon is no longer painful at rest. It is when the tendon tolerates the load of the sport, through a defined volume and intensity, across several sessions.
Adults with chronic tendinopathy that has not responded to at least one structured course of conservative care — Achilles, patellar, rotator cuff, lateral/medial epicondyle, proximal hamstring, gluteal.
Patients with mechanical tendon pathology — retracted full-thickness rotator cuff tear, ruptured Achilles, symptomatic calcific deposit requiring decompression — are evaluated for surgical care through Dr. Rahman's surgical practice.
Depending on modality volume, duration, and whether an ultrasound-guided procedure is included.
